For much of the 18th and 19th centuries, "cleavage enhancement" was mainly achieved through adjustments in corseting. I once read some actress explain how 18th-century type "stays" can give almost the flattest woman some cleavage (and create a little space in front, accessible in low-cut gowns, where she can tuck letters or small objects away).
